E3 Diabetic Care Program
The goal of the E3 Diabetic Care Program is to Engage, Educate and Enhance the quality of life for pre-diabetic and diabetic persons by providing a program which will improve overall awareness and compliance.
What does this mean?
Engage – We are here to supply you with select diabetes testing supply needs shipped directly to your door. Our goal is to ensure you have the right product, delivered on time, so you never have to worry about running out of supplies. (Please note that a prescription is required for supplies).
There is an immediate cost savings allowing you to receive diabetic testing supplies at no cost.
Educate – We provide education and support to aid you in better managing your diabetes, helping you to prevent complications and maintain optimum health.
Enhance – The value of little to zero cost diabetic testing supplies, coupled with customized education and support, drives behavior and lifestyle changes to enhance your quality of life. This leads to greater compliance with diabetic testing for improved health outcomes.

Prevent Type 2 Diabetes Program
A lifestyle change program proven to prevent or delay type 2 diabetes. It includes:
- A CDC-approved curriculum
- The skills you need to lose weight, be more physically active, and manage stress
- A trained lifestyle coach to guide and encourage you
- Support from other participants with the same goals as you – and fun
- A year-long program with weekly meetings for the first 4 months, then twice a month for 2 months and once a month for second 6 months
You may have prediabetes and be at risk for type 2 diabetes if you:
- Are 45 years of age or older
- Are overweight
- Have a family history of type 2 diabetes
- Are physically active fewer than 3 times per week
- Ever had diabetes while pregnant (gestational diabetes) or gave birth to a baby that weighed more than 9 pounds
Employers’ Advanced Cooperative on Healthcare (EACH) has offered this class for the past 3 years and successfully graduated 95% of the participants, with an average weight loss of 6%.
The Prevent Type-2 Diabetes program is a yearlong program that takes commitment from each participant.
